Understanding MSRA: Who Takes It and How It Differs…
The MSRA is a computer-based assessment used during recruitment for several UK medical specialty training programmes. It tests professional judgement and clinical problem solving, with the score contributing to specialty selection. It is not a postgraduate qualification and does not replace a specialty examination such as the MRCGP.
What is the MSRA and who needs to sit it?
The Foundation Programme covers two years of supervised medical training, which is known as F1 and F2. Doctors move through different:
- Clinical placements
- Work with NHS teams
- Build practical skills
- Complete the assessments linked to their training
The important point here is that getting a place involves several stages.
The UKFPO describes the recruitment process as an eight to nine month cycle. It includes nomination or eligibility, the Oriel application, pre-allocation reviews, Foundation School allocation, programme preferences, employer matching, and the final preparation for starting F1.
That length catches some applicants off guard.
The MSRA is designed for doctors applying to selected UK specialty training programmes, with eligibility depending on the specialty and recruitment round. NHS England currently lists programmes including:
- General Practice
- Core Psychiatry
- Clinical Radiology
- Core Surgical Training
- Anaesthetics
- Obstetrics and Gynaecology
- Ophthalmology and several other pathways
For an applicant, the practical point is quite simple. You do not decide to take the assessment merely because you are interested in one specialty. You need to check the recruitment guidance for your chosen programme and confirm whether eligible applicants are invited.
The assessment has two parts called Professional Dilemmas and Clinical Problem Solving. The current structure gives 95 minutes to Professional Dilemmas and 75 minutes to Clinical Problem Solving, with the two papers delivered as one assessment.
Clinical Problem Solving uses clinical scenarios that require candidates to apply knowledge, interpret information and select appropriate responses.
That distinction matters when preparing.
Someone can have strong clinical knowledge yet lose marks through poor judgement in a professional scenario. Another candidate might know the clinical material well but struggle with the pace required to process several pieces of information quickly.
How does the MSRA work during specialty recruitment?
The assessment functions as part of a recruitment process rather than as a qualification that sits on your professional record. NHS England describes it as an assessment of selected competences from the relevant Person Specification, using clinical scenarios as the basis for questions.
For some specialties, the score can play a major role in shortlisting or later selection stages. The exact process varies between specialties, so applicants should read the guidance for the recruitment round they are entering.
There is another detail that international medical graduates should watch closely. Previous scores may not automatically transfer between recruitment rounds or specialties. For example, current specialty guidance states that some recruitment processes require eligible applicants to sit the assessment for that round, rather than carrying forward an earlier result.
This can change the way you plan your preparation.
What does the assessment actually test?
The assessment is not built around memorising a single list of diseases. Clinical Problem Solving asks candidates to interpret clinical information and reach an appropriate response. Professional Dilemmas asks candidates to judge workplace situations involving patients, colleagues and professional responsibilities.
A useful preparation mindset is to ask what the scenario is really testing.
For example, imagine an F2 doctor notices that a colleague appears exhausted after several consecutive shifts and has started making documentation errors. The question may not reward the response that sounds most heroic or confrontational. It may test whether you can recognise a patient safety concern, respond professionally and escalate the issue through an appropriate route.
That kind of reasoning requires more than factual recall.
MSRA Vs. MRCGP and why the distinction matters
MSRA Vs. MRCGP is not really a comparison between two versions of the same examination. They serve different purposes at different stages of a doctor’s career.
The MSRA supports selection into certain specialty training programmes. The MRCGP forms part of the assessment pathway for doctors completing UK GP specialty training. The RCGP states that its three components are the Applied Knowledge Test, Simulated Consultation Assessment and Workplace Based Assessment.
| Area | MSRA | MRCGP |
| Main purpose | Specialty recruitment | GP specialty training assessment |
| Typical stage | Before entry to selected training programmes | During GP specialty training |
| Main focus | Professional judgement and clinical problem solving | Knowledge, consultation skills and workplace performance |
| Qualification | Recruitment assessment | Part of completing GP specialty training |
| Administered through | NHS specialty recruitment process | Royal College of General Practitioners |
The difference becomes clearer through a common applicant situation.
Suppose an international medical graduate has completed PLAB and is preparing for UK specialty recruitment. They may see General Practice listed among programmes using the MSRA and assume that passing it means they have completed the GP qualification pathway.
That assumption would create confusion.
Passing the recruitment assessment can support entry into the relevant training process, yet it does not mean the doctor has completed GP specialty training. The later GP training pathway includes the required MRCGP components and progression through training assessments.
Does passing the MSRA mean you have completed GP training?
No, passing the recruitment assessment does not mean that GP specialty training has been completed. The assessment serves a recruitment function, so its role comes before or around entry into the relevant training pathway rather than acting as the final GP qualification.
The RCGP states that doctors undertaking UK GP specialty training complete the AKT, SCA and Workplace Based Assessment as components of the MRCGP. Completion of the required GP training and assessments then supports the process leading toward a Certificate of Completion of Training.
This is one reason applicants need to separate recruitment preparation from later training preparation.
The skills overlap in places, especially clinical reasoning, professional judgement and understanding of NHS practice. The purpose of each assessment remains different.
How should doctors prepare for this type of assessment?
Preparation works better when it reflects the actual assessment structure rather than treating every question like a traditional medical knowledge test.
A sensible preparation routine can include the following areas.
- Practise professional judgement scenarios with careful attention to escalation and patient safety.
- Build speed when processing clinical information under timed conditions.
- Review common NHS workplace situations that may appear in professional scenarios.
- Practise Clinical Problem Solving questions without relying on memorised answer patterns.
- Review official guidance and the current recruitment information for your chosen specialty.
- Use timed practice to identify whether errors come from knowledge gaps, rushed reading or poor decision-making.
Official NHS guidance provides a practice paper and explains the two-part assessment structure, which gives candidates a useful starting point before moving into more intensive preparation.
The most useful preparation is rarely about doing hundreds of questions without reviewing them. A candidate who spends time asking why an answer was inappropriate can learn more from one difficult scenario than from simply recording another practice score.
What should international medical graduates check before applying?
International medical graduates should check the eligibility requirements for each specialty and recruitment round rather than relying on advice from an older application cycle.
Recruitment arrangements can change, including testing windows, score handling and specialty-specific selection methods. NHS England guidance for the 2026 recruitment cycle, for example, described separate assessment windows for some applicants and stated that the February window was reserved for applicants applying to Core Psychiatry and or General Practice.
That means preparation should start with the recruitment rules, followed by a realistic assessment plan.
For candidates coming through PLAB and then moving toward UK specialty recruitment, understanding where the assessment fits can remove a lot of unnecessary confusion. The MSRA is a selection assessment, not a replacement for later specialty training assessments.
Final thoughts
The main point is the purpose behind the assessment. The MSRA helps recruitment teams assess applicants for selected specialty training programmes, whereas the MRCGP sits within the longer GP specialty training pathway.
For anyone planning a UK medical career, keeping those stages separate makes preparation much easier. Before booking study time or choosing resources, check the current recruitment guidance for your intended specialty and understand exactly where the MSRA fits into your application.
